Cardiac Arrest during Anesthesia for Patient with Pheochromocytoma


Abstract
The anesthetic management of patient with pheochromocytoma presents many difficult problems even when a preoperative confirmed diagnosis has been made. This case illustrates a sudden cardiac arrest during anesthesia and operation for biopsy and resection of the tumor in 49 year-old male patient who was suspected to suffer from right adrenal gland tumor. The nature of the tumor was revealed as pheochromocytoma later.
Since the patient showed normal range of blood pressure, no preoperative management was done. Thiopental sodium 5mg /kg and succinylcholine 1mg /kg were used for induction, followed by N2O, O2 and enflurane for maintenance.
Fifteen minutes after the start of operation, manipulation of the right adrenal gland lead to sudden hypertension and sinus tachycardia. Ventricular fibrillation followed and it was treated by D.C. cardioversion. After tumor resection, low blood pressure was controlled by dopamine dripping.
